Nedlands Community Care

Nedlands Community Care (NCC) has been your local trusted home support provider for more than 35 years. Funded by the Commonwealth Government Home Support Program (CHSP) and the City of Nedlands, NCC supports services needed to allow senior residents in the community to continue living safely in their own homes.

The Commonwealth Home Support Program allows NCC to provide both in-home support activities and group social support services through the City’s Waratah Social Club.

Self-funded seniors who are not eligible for government subsidised CHSP services, but who would like to receive NCC services or participate in the City’s seniors’ activities are welcome to contact us for further information.

Nedlands Community Care is located at 97 Waratah Avenue, Dalkeith and is open from 8am - 4pm Monday to Friday.

For information about any of the services provided by NCC please contact us on 9386 6170 for further information.

To check whether you are eligible for CHSP services, contact My Aged Care on 1800 200 422 or log on to the myagedcare.gov.au website and complete an online assessment. This can be completed personally, or by a family member or friend.  Alternatively, you can call NCC on 9386 6170 and speak to our friendly staff for assistance.

Once you have completed the online assessment, you will be contacted by an assessor from the Regional Assessment Service (RAS), to further discuss your needs. You may like to prepare questions in advance of this discussion and you may have a support person of your choice to accompany you at this assessment.
